To see the image on the front and back of the tag, the most trouble-free method is to create separate STLs for the tag and the picture. In Prusaslicer, cut the tag part with the picture as negative space, then position the picture at the same location as the negative space cut. This prints fine, but the two parts don't bond so well, it may separate under rough handling.
This model combines the picture with two layers in the centre to keep the picture parts tightly bound to the top and bottom parts of the tag. For this you have to do a cut out in the tag part, to make space for the two inner layers.
Print in PETG for more flexibility than PLA.
The card is 1 mm tall (5 layers). Ensure that top plus bottom layers is more than card thickness. This is equivalent to 100 infill for this thin part.
