I created this so I could mount a Noctua NF-P12 fan on the back of my rack to blow air towards some of my components, like my network switch.
The mini PC in my rack is unable to provide 12V PWM power to an external fan, so I added mounting holes for a temperature based fan controller I found on Amazon (https://a.co/d/6BruVtL). This fan controller does not come with a power cable, so if you need to buy one (like I did), this is the power supply I bought (https://a.co/d/0PcKPrQ). Then you just need two small lengths of wire to connect the terminals.
The mounting screws that came with my Noctua fan worked well straight out of the box, so the only additional hardware needed is 4 m3 screws and 8 m3 nuts. These are used to mount the controller to the plate. I recommend putting two sets of nuts on the bolts, one between the controller and mounting plate, and one to hold the controller to the bolt. The controller has some protruding components on the backside, and I didn't want to damage it by squeezing it against the mounting plate. And of course you will need mounting hardware to mount this to your specific rack.
For my print profile, I used with 3 wall loops, 5 top and bottom shell layers, and 25% infill. No supports or brim needed. I added the extra strength both because I printed in PLA instead of PETG, but also because I didn't want the fan to transmit unnecessary vibrations.
