Using FreeCAD simply put in Name, Address (short, 1 line, like city and zip) phone number and ICE (in Case of Emergency) Number then select either the "blank-Wcut" or the "blank WOcut", each of the "Extrude" text parts and export an an STL then slice and print that STL (see instruction pic).
Best printed in PET-G and in 2-color (at .2 mm layer height, color change is at layer 16).
