Swiss Cheese Model

What it is?

The Swiss Cheese Model is a causation model utilized for risk analysis and management across many fields and industries. Its purpose is to illustrate that catastrophic failure or accident are often caused by multiple smaller failures. 

Every slice of cheese in the model is unique, featuring random holes of varying sizes and numbers. The holes is represents lapses or areas where potential hazards exist. Each slice represents a measure taken to minimized risk and it viewed as a line of defense against failures. When all of the cheese slices are put together, they collectively represent the defense against risks. 

The risk of threat occurring is mitigated by the differing the layers of defense (Cheese). Sometimes these hole align, creating a path that extends all the way through, resulting in a major failure. This demonstrates that failures are not caused by a single event, but rather a combination of events. The goal of the model is to minimize risk by identifying and addressing weak points, helping to understand why failures occurs despite efforts to prevent them.
