That is the replaceable knob in a case if we lost the original. That one have three different size ( In terms of how tight it sits on the rod axis) depends on how your printer prints.
The holes are made for you to see the colour - because through the plastic it is difficult to see ( Even if it is a transparent).
I have used successfully the medium size with my 0.6 MM nozzle.
