Small Extruder Knob Extended Shaft (Remix)

I liked the design of this knob from user @MMBavaria quite a lot, but needed some extra clearance for the idler wheel screw on the stock Ender 3 extruder piece. I added 5 mm extra to the shaft area, and flattened the top surface.
