Our safety department didn't think zip ties and orange streamers were good enough so they wanted us to get something more official for locking out valves.  The official products sold on sites like Grainger are also plastic and would be pretty easy to bypass or tamper with (just google “ball valve lockout”) and can cost $30-60 each.  We were looking at close to $1000 in stuff…

These were acceptable for our safety department so they ordered me 8 spools of PETG.  Here's the files for anyone in the same boat.

The small one does ½" to 1", the medium does 1½" to 2", large does 2" to 3" Spears and Asahi brand/style valves.

The two holes on the outer shell are spaced so you get twice the adjustment positions.

Print them with the big flat side down.  I did 6 perimeters with a .4mm nozzle or 4 with a .6mm (.2mm and .25mm layer heights respectively) and 25% gyroid infill.  PETG worked for our purposes because it's strong enough and they are not exposed to the elements or actual chemicals.  Use what your needs require.

Weights of printed pairs with my settings: Small - 220g, Medium - 455g, Large - 643g

Like existing LOTO devices, these are not indestructible or idiot-proof.  Nothing beats a good safety culture, training, LOTO plan, Confined Space plan, and communication.  Use at your own risk after consulting with all relative parties at your organization.
