The cylindrical pill container end cap/hex-nut has a left-hand thread that few people undo successfully. Most try to unscrew the end nut assuming it is a right-hand thread. Of 20 people who tried to open this puzzle, only two were successful. Both are doctors. The center nut has a standard right-hand thread that is a “red-herring” (distraction).
The center nut is printed concurrently on the bolt and cannot be removed, but turns freely. One of the pleasures of life is putting the skinny hex in a vice and breaking the center nut loose with a bit of cracking noise. (See photo.)
