I have some of those ceramic toilet paper holders that have a concave back and won't let you use large roles of toilet paper without rubbing and not spinning properly. This likely won't work for all toilet paper holders but I hope this helps others that have a similar holder.
The grub screws hold the extender in the original role holder holes.
I printed my copies on a stock PRUSA Mini without any issues. I would recommend using supports and printing the screws with 100% infill otherwise they tend to break.
I've included the Fusion 360 files so you can modify to suit your own needs.
