I made this model to sit on top of a telescoping fiberglass pole tipped with an ACME 3/4-5 thread, a typical attachment mechanism for "painter poles".   I am using this 20' fiberglass push up pole:  https://www.amazon.com/dp/B0FP567P8S

The model has one position for attaching a vertical telescopic antenna whip and three positions for attaching three telescopic antenna whips as elevated radials.  The attachment points are sized for M10x1.5 threaded heat inserts.  Use of a M10 to 3/8-24 adapter would  allow use of whips using that thread.

There is a provision for attachment of a SO-239 4 hole panel mount adapter.  The SO-239 screw holes are sized for M3 threaded heat inserts.

Wired  3/8" / M10 crimp ring terminals are connected to the SO-239 and get fitted between antenna whip & base.

The model allows the angle of the three radial attachment points to be oriented as desired. Radial angle can affect SWR readings.  The radial antenna mounts are held in place with printed screws.

I also included a model that serves as guy line tie off point for the fiberglass push up pole I used.  It is sized to slip past the two upper locking rings.

Fusion 360 source and .STEP files are included as well as .STL files.

This model is an evolution from my earlier pole topper antenna base.  I have used it with two radials and wanted a version with three for a better pattern:
