As chain started falling of I looked for a solution, but most of guider were made for mountain bikes that has different diameter.
This model is heavily inspired by https://www.printables.com/model/146777-mountain-bike-chain-guide
Mount is for 39mm dia, installation is simple just few parts. For mount I prefer hex screws as it's difficult to reach them with straight tool.
You will need 4 x M5x20mm screws and nuts.
