I made this quick release plate for my Velbon DF-60 / DF-61 tripod, to make it work with my Hadley telescope and the tripod adapter made by Gyoenastaader.

Use 10-24/0.5 or M5 screws. The diameter of the holes and counterbore hole are 6mm and 8.5mm, the counterbore depth is 5mm.

STEP files are also added, so you should be able to edit this part in you 3d-modelling software to fit your choice of screws.
