Most telescoping ladders come with a plastic cap on the top to prevent water and debris from falling into the rails. Those unfortunately come off easily and then get lost easily. You can find replacements fairly inexpensively, or you can print them. This design is for ladders with a 28mm top hole.
I put bumps on the inside of these to get a tight fit, and these do fit fairly tightly. I printed mine in PLA but if your ladder spends a lot of time outside then you may want a more UV-resistant material.
